A newly released National Transportation Safety Board (NTSB) investigation into the fatal Hunts Point explosion highlights how quickly routine maintenance work can become catastrophic. According to the NTSB’s final report, the probable cause of the explosion was unapproved hot work conducted above a cargo tank. The tank contained sludge that produced flammable methane gas which was ignited by a propane torch being used inside the cargo tank.
The NTSB investigation offers an important reminder for maritime workers, vessel operators, and employers: dangerous conditions can develop during ordinary maintenance tasks. It also highlights the importance of understanding the dangers of hot work and ensuring proper safety measures are followed.

The Hunts Point was a 288-foot liquid cargo vessel owned and operated by the New York City Department of Environmental Protection. It was designed to transport treated sewage sludge from wastewater treatment facilities to a dewatering facility. The vessel had six cargo tanks capable of carrying up to one million gallons of liquid sludge.
The explosion occurred on May 24, 2025, while the vessel was moored at New York City’s North River Wastewater Treatment Plant. One crew member was killed and three others suffered injuries. The NTSB estimated that the vessel sustained more than $10 million in damage.
On the day of the explosion, approximately 40,000 cubic feet of sludge remained aboard from the previous day’s operation. The sludge had become too viscous for the vessel’s pumps to move effectively. As a result, the crew began a process known as innage reduction, which involved adding water to the cargo tanks to thin out the material.
This process had not been done aboard the Hunts Point in five years. Over that period, pipe plugs had become rusted and seized. Several bolts also broke while crew members attempted to remove the flanges securing them. The broken bolts eventually became the focus of the maintenance work that occurred immediately before the explosion.
According to the NTSB report, the chief engineer and an assistant were working near a cargo tank attempting to remove the broken bolts. The chief engineer ignited a propane torch to heat one of the broken bolts. The vessel’s master had reportedly warned the chief engineer twice not to use the torch on deck. The report says that he separately instructed the assistant engineer to remind him not to use it near the cargo tanks.
Shortly afterward, the assistant engineer reportedly saw the chief engineer ignite the torch. A yellow flame then appeared near openings in the cargo tank hatch cover. Seconds later, the assistant engineer reports hearing sounds coming from inside the tank just before the explosion occurred. The blast caused the cargo tank to expand upward damaging cargo piping, catwalks, stairs, and other vessel components.
The chief engineer was thrown from the vessel and was later found between the dock and a wooden fender. He was pronounced dead at the scene. The assistant engineer and two other crew members survived with injuries.
One of the most important lessons from the investigation is that treated sewage sludge can still produce hazardous gases. The NTSB reported that the sludge aboard the vessel was capable of producing gases, including:
The report explains that these gases could collect inside the cargo tanks. Methane, in particular, is flammable and explosive in certain concentration ranges. The sludge had been treated to remove methane and stabilize the material, but treatment did not eliminate all potential hazards. The NTSB noted that the remaining material could still produce flammable gases as organic matter continued to decompose.
Because the sludge had remained in the tanks from the previous day’s operation, the concentration of flammable gases likely started building up again. That created a potentially dangerous environment in and around the cargo tanks.
Hot work refers to work that creates a flame, heat, sparks, or another potential ignition source. Examples are welding, cutting, brazing, grinding, torch use, and certain other maintenance activities. These procedures often require a hot-work permit. Hot-work permits are designed to identify and control ignition hazards before work begins.
The NTSB found that New York City’s DEP policies at the time of the explosion classified the use of a torch or welding equipment on its vessels as hot work. The investigation found that the DEP’s procedures required a permit before hot work began aboard its Marine Section vessels. Investigators also found that the crew knew open flames were prohibited on deck.
The report also identified an important training issue. The chief engineer’s training records did not indicate specific training concerning hot work or the risks of gases produced by the sludge. The annual safety training conducted in 2024 included a reference to the importance of assigning a fire watch but did not otherwise address hot-work procedures or the risks associated with sludge gases.
